Case Summary:

42 yr old male, presents with wife. C/O Headache, dizziness after workplace injury “day before yesterday.” Hit on top of head. Seen at home hospital, treated for closed head injury and laceration to top of head sutured, sent home with Head Injury information sheet. No imaging done. Triaged to streaming area

Case Summary:

66 year old male at work with sudden collapse. Decreasing level of consciousness en route to ED.
- Patient given 20mg midazolam by paramedics for intubation - unsuccessful.
Pt deteriorates in ED, requires intubation and starts to seize.

Pre-hospital Notification: 18 year old male. Dived of a cliff into shallow water. Arrival by helicopter – scene response. C-spine secured by EHS.

On Arrival/History: 18 year old male, Drinking at bush party, dive from 15 foot cliff into shallow water. Came up face down, face underwater for 5 minutes before rescued by friends.

Closed Head Injury. Pedestrian struck in crosswalk yesterday. Non surgical diffuse axonal injury. CT scan diffuse swelling and petechial hemorrhage. No mass or blood.
Cervical collar in place. Also cast on right leg post closed reduction ankle fracture.
Intubated. BP 160/80 PR 85 SpO2 96%
Sedated with proposal infusion at 40 ug/kg/min.
Orders to maintain RASS goal -4 and Systolic BP between 140 and 180.
Prn Hydralazine 2-4 mg prn and Labetolol 5 to 10 mg.

Social Hx: Family gone home. First assessment post shift change.
