Scenario Information


     Case Summary:

     ­Baby born at 38 weeks +4 gestation. His mother, who is now G-2, P-1, had previously attempted a home delivery and labored for over 26 hours before she reconsidered and went to the hospital. She experienced SROM approximately 20 hours prior and developed chorioamnionitis; the fever and uterine pain lead her to seek medical treatment. While in the labour suite, it was determined that she was GBS+ and then quickly received a dose of IV antibiotics, just 30 minutes before delivery.  Babe appears to be having difficulty breathing right after being placed on skin-to-skin bonding; now brought to the warmer. NICU was called to second in.

Scenario Information


     Case Summary:

     ­8 yr old boy who had a FOOSH on left hand, midshaft radius/ulna fracture requiring reduction under procedural sedation. Brought from school by EMS and EMS placed an IV in the right arm. Has a history of mild intermittent asthma (viral-induced). No current meds, no allergies, no previous sedations/GA (no previous anesthetic reactions). No family history of anesthetic reactions. Had a URTI 3 weeks ago, required Ventolin for 1 week (none over past 2 weeks). Has not received any analgesics.

  ­Patient Description:
Name:NBF
Age: 37 weeks gestation, 1 hour old
Weight: 2863 g
Immunizations: nil
Hx of current condition: - normal pregnancy except for PIH and evolving HELLP syndrome
- Labour induced, ROM for 2 hours, FHR normal, 3 doses maternal penicillin, GBS unknown, meconium present, precipitous SVD
- APGARS 7 at one min, 8 at five min, 9 at ten min
- Babe initially hypotonic but responded well to stimulation, no other resus required
- Indrawing and grunting since 3 minutes of age
- Observed for possible TTN in SCN in isolette
- Ongoing respiratory distress with increasing oxygen requirements due to low O2 sats

Social Hx: no concerns


Diagnosis: respiratory distress
